Sales Rep Appointment Request

This form is for Sales Representatives that are looking to sell products or services to BIKE513. Please read all of the information below before filling out the form so you understand what Geoff's (owner) expectations are.

BIKE513 is Mechanic owned, Geoff is scheduled in the shop and on location with our First Responder accounts.  Appointments are a not recommended, but required. 

Appointments with Geoff are available Tuesday though Friday 9:30am - 11:00am. After submitting this form you will receive a confirmation for your appointment within 48 hours. If you do not get a reply, then he is unavailable to meet.

Punctuality and communication will dictate Geoff's availability on the date of appointment.  Tardiness will result in a failure to conduct future business.

This request form must be filled out for each visit. No walk-in appointments available.
